The Commissioner of the Tennessee Department of Correction, Frank Strada, has resigned following the botched execution of convicted murderer Christa Pike, who survived two injections of the lethal drug pentobarbital.
Tennessee Governor Bill Lee announced Strada’s resignation on Saturday, saying the commissioner would leave his position later this month. The governor has also ordered an independent review of the failed execution.
Pike, 50, was left unconscious and on a ventilator in a hospital following Wednesday’s failed lethal injection at the Riverbend Maximum Security Institution in Nashville, according to her lawyers. Hospital staff were working to save her life and clear the two doses of pentobarbital from her system.
Pike was sentenced to death for the 1995 murder of 19-year-old Colleen Slemmer, a crime she committed when she was 18.
It was the second failed execution in Tennessee in 2026. In May, the state halted another execution after medical staff were unable to establish intravenous access.
Lee, who had previously denied Pike’s clemency request, has suspended further executions in Tennessee for the rest of the year while the independent review is conducted.
In a statement, Lee thanked Strada for “his willingness to put the interests of Tennesseans first during this difficult moment.”
Strada, however, said he believed the Department of Correction had carried out its responsibilities in accordance with established protocols, adding that stepping aside was in the best interest of the state as the review proceeds.
Pike’s failed execution has drawn renewed scrutiny of Tennessee’s lethal injection procedures, with her lawyers alleging that difficulties in establishing intravenous lines contributed to the failure.
Had the execution been completed, Pike would have become the first woman executed by Tennessee in more than two centuries. Nationally, 18 women have been executed in the United States since 1976, according to the Death Penalty Information Center.
