The Taraba State Government has introduced digital health management system to tackle fragmented patient records, missed medical appointments and gaps in follow-up care across healthcare facilities in the state.
The Integrated Client Management Application, being developed in partnership with Jhpiego, will enable healthcare workers to track patients’ treatment histories, receive alerts on missed appointments and monitor medication adherence across multiple health programmes.
The platform is expected to cover HIV treatment, antenatal care, labour and delivery, immunisation, nutrition, outpatient services and community healthcare, replacing the limitations of a disease specific system with a more coordinated approach to patient management.
Speaking at a meeting on the initiative, the State Commissioner for Health, Dr Bodiya Buma, said the transition to an integrated digital platform represented a significant step towards improving healthcare delivery in the state.
Dr. Buma, urged the implementation team and developers to address the shortcomings of the previous application to ensure that, the new system delivers the desired results and remains sustainable.
He commended Jhpiego for selecting Taraba as one of the first five states in Nigeria to implement the initiative and pledged government’s support for its successful deployment.
The commissioner, however expressed hope that the initiative would demonstrate the effectiveness of integrated digital health technology and provide a model for adoption by other states.
The Team Lead of Jhpiego, Mr. Emmanuel Atuma, explained that the new application was the third phase of the organisation’s digital health intervention in the state.
He said the initiative builds on an earlier case management application introduced under the USAID-funded Reaching Impact Saturation and Epidemic Control (RISE) Project to improve the management and follow-up of clients, particularly those receiving HIV services.
According to Mr. Atuma, the new platform would consolidate patients’ information across different healthcare services under the principle of “one app, one client record,” giving healthcare providers a more comprehensive understanding of individual treatment histories and care needs.
He also added that the application would feature automated appointment reminders, alerts for missed clinical visits, medication adherence monitoring, offline access and data analytics to help health authorities identify service delivery gaps and make informed decisions.
Mr. Atuma said the current phase prioritised sustainability and state ownership, with the Ministry of Health and relevant state agencies expected to take leading roles in implementation.
He identified the training of frontline health workers and other users as a critical component of the initiative, noting that building local capacity would enable the state to manage and sustain the platform beyond the period of partner support.
Atuma further commended for the foundation for its commitment to advancing digital health solutions in the state.
In his remarks, the Director of Public Health, Dr Abe Agbu, praised Jhpiego for its previous interventions in Taraba, and expressed confidence that the new system would improve coordination among healthcare providers and strengthen continuity of care.
“Beyond patient tracking, the application is expected to reduce the paperwork associated with healthcare delivery, allowing medical personnel to spend less time managing records and more time attending to patients.”
The initiative, supported by the Patrick McGovern Foundation, marks a further effort by the state government and its development partners to use digital technology to improve healthcare coordination, strengthen patient monitoring and build a sustainable health information management system.
