The hospital, in an internal memo dated September 10, 2026 and signed by its Director of Clinical Services/Chairman, Medical Advisory Committee, Dr O. Lawal, and obtained exclusively by LEADERSHIP, said the serious adverse events were identified by its Pharmacovigilance Committee.
According to the memo, the two patients were adults who were “otherwise relatively stable” before the administration of the drug. The hospital directed all clinical units and departments to heighten surveillance of the use of Ceftazidime sourced from outside the hospital, particularly Fortcef Injection 1g.
The affected product was identified as batch number PEZIE001, manufactured in May 2025 and due to expire in April 2028, with NAFDAC registration number C4-0247.
The hospital said the cases had been reported to the National Agency for Food and Drug Administration and Control (NAFDAC), while clinicians were directed to promptly report any suspected adverse drug reaction to the Pharmacovigilance Committee.
The development followed a viral voice note purportedly recorded by a nurse at the hospital, in which concerns were raised over deaths allegedly occurring after patients were administered Fortcef.
The internal memo, however, did not support the allegation of four deaths contained in the viral claim. Rather, it specifically documented serious adverse events involving two adult patients.
The hospital’s decision to issue an internal surveillance alert and report the cases to NAFDAC indicates that the suspected adverse drug reactions are being subjected to pharmacovigilance investigation.
